Providing Dental Care for Older Adults

LIVE WEBINAR: Thursday, October 7, 7:30pm

Description:

The World’s population is aging, and an increasing number of people are presenting to our community dental offices with the positive and negative effects of old age. A growing number of older adults are successfully aging at home, and living active lifestyles. The prevalence of chronic conditions and disabilities has declined over the past seventy years due to improvements in diet, an increase in physical activity, and reduction in smoking. In addition medical advances in orthopedics, implants/transplants, and cardiac pacemakers have helped people live longer and healthier. However many older adults suffer chronic disease and disabilities including cardiovascular disease, cancer, arthritis, and senile dementia. In addition, the provision of dental care for older adults is affected by polypharmacy and xerostomic effects of many medications.

Older adults are retaining teeth longer and have experienced sophisticated dental care over their lifetime. They have invested a considerable amount of time and money in their mouths and expect to retain their teeth, implants and dental prostheses for a lifetime. However, for many older adults, poor oral hygiene predisposes them to gingivitis, periodontitis, denture stomatitis, and especially dental caries. Poor oral health and tooth loss often results in discomfort, poor aesthetics, halitosis, compromised mastication, and these combined decrease quality of life.

The consequences of missing teeth are compromised aesthetics, phonetics, mastication, and occlusion. The loss of a maxillary incisor is just as much an aesthetic concern for an older adult as a younger adult. The options for replacement of teeth is no different from those who are younger. However, frailty (physical and cognitive) pose significant limitation on a patient’s ability to undergo a complex treatment and comply with maintenance recommendations. The provision of dental care for older adults sometimes involves consent for treatment from others (family and guardians), determining the best environment to provide services, and the support from others for daily oral hygiene, and maintenance of the dental prostheses. Increasingly, dental professionals must work with fellow health care providers (physicians, social workers, and community nurses) to better serve their elderly patients.

Learning Objectives:

  • Be aware of the impact of aging on general health, mobility, cognitive ability, and oral health.
  • Be able to apply a rational approach to diagnosing oral disease and developing appropriate treatment plans to restore dentitions affected by tooth loss, dental caries and periodontal disease for older adults.
  • Understand the need and have the skills to obtain appropriate levels of consent for dental care, and communicating with relatives of patients and other health care providers.

The Way We Breathe Influences Facial Growth and Development

LIVE WEBINAR: Wednesday, October 27, 7:30pm

Description:

How we breathe affects facial growth and development. Soft tissue such as the tongue, lips, cheeks, tonsils and adenoids influences growth. Certain habits, compensations, and airway-related issues can contribute to adverse skeletal changes.

In this lecture, we will discuss how soft tissue and breathing patterns influence skeletal growth. We will examine similar malocclusions in children and adults, and discuss how the treatment plans differ, depending on the age and skeletal maturation of the patient. For adolescents, our treatment plans will focus on growth modification. For adults, we will explore both surgical and non-surgical options, including adult TAD expansion, as we derive the ideal comprehensive treatment plan.

Learning objectives:

  • Recognize how dysfunctional habits and patient compensations lead to common malocclusions
  • Understand interdisciplinary treatment strategies for early intervention
  • Learn surgical and non-surgical treatment strategies for adult treatment

Chasing Nature: Techniques for ‘Ultra-mate’ Biomimicry

LIVE WEBINAR: Saturday, December 4, 3:00pm

Synopsis:

The delegate will review strategies for completing esthetic Class V restorations and isolation concepts in a distal free end extension case without an anchor tooth for rubber dam placement in an everyday clinical workflow.  Bevel concepts will be discussed.  Layering concepts in posterior esthetic composites will be reviewed in terms of sequence, with an emphasis on tint placement and timing for realistic-looking outcomes.  A review of esthetic parameters relative to both pink and white esthetics will be completed with strategies given for balancing asymmetries using Gemini SuperPulsed dual-wavelength laser-guided gingival recontouring in cases with osseous recontouring required.  A review of biologic width parameters to be re-established is planned along with myths debunking the ability of lasers vs. retraction cord in their role to cause recession – which causes more?  A review of incisor hypomineralization, its etiology and current best practice protocol for staged minimally-invasive treatment will be reviewed.  Finally, a review of anterior esthetic layering will be undertaken with a review of “how much to prepare to achieve a color improvement.”

Learning Outcomes:
1) The attendee will review everyday restoration isolation challenges and receive strategies to overcome these.
2) The attendee will review a predictable posterior layering protocol as part of an adhesion strategy that will minimize post-operative sensitivity and maximize microshear bond strengths.
3) The attendee will learn how to manage limited osseous recontouring in the realm of gingival recontouring for balancing gingival asymmetries in the smile zone.
4) The attendee will review the components of the SkoolToolz Hu-Friedy kit and how this can simplify their everyday direct and indirect restorative armamentaria.
5) The attendee will review the importance of proper light intensity and duration during curing.  How much is enough?  How much is too much?  Is there such a thing as too much?
6) The attendee will review the best practice protocol for the staged treatment of incisor hypomineralization.
7) The attendee will review anterior layering strategies for optimal esthetic.

“Take your team from surviving to thriving in 2021 – You deserve it!”

Leadership and building your team are looking very different today, which is mainly due to COVID- 19. There have been a number of studies done regarding leadership and what it looks like in this new world….the results have clearly indicated a transformational cultural, shift. This calls for a change in the way we lead and develop our team. During this webinar we will discuss the following topics:

1) Building your team; how to “lead” rather than manage your team
2) Identifying your leadership style

Current Concepts and Future Trends in Endodontics (Live Webinar held Nov. 5, 2020) On-Demand Recording Not Available
Endodontic treatment is one of the most challenging procedures in dentistry as the complexity and variability of the root canal system can often result in procedural mishaps and treatment failures. In recent years, many advances in endodontic technology have helped clinicians perform safer procedures and with better long-term outcome. Utilizing operating microscopes and Cone Beam Computed Tomography (CBCT) are gold standards in endodontics. New technologies such as guided endodontics utilizing CBCT-/Computer-driven approaches and irrigation devices using multi-sonic technology may hold promise to enhance our treatment outcomes.

Our continual understanding of the biology and the causes for endo-restorative treatment failures are helping us move away from a more traditional approach to endodontic treatment and guide us towards a more conservative approach, specially when it comes down to access cavity preparations and canal debridement. As the research and technology continue to mature in the field of endodontics, we should expect a trend towards reduction in treatment failures and faster healing rates, and an increase in long-term survival of natural dentition.

This lecture is an overview of all steps involved in the initial endodontic treatment and a review of common practices according to our current understanding of the biology and available techniques and materials. (1.5 credit hours)

Guest Speaker: Dr. Mahmoud Ektefaie
Certified Specialist in Endodontics

Dr. Mahmoud Ektefaie received his DMD from the University of British Columbia in 2003 and his Certificate in Endodontics and M.Sc. in Oral Biology from Temple University (Philadelphia, PA) in 2008.
